2. Depression- Anxiety (Depends on severity) Certain carriers acceptable.
3. Pregnant -currently
4. Rheumatoid arthritis
5. Any serious surgery in the last 2- 5 years. I.e. . Heart, Internal organs, Spinal, Brain, Eye etc..............
6. Cancer within the last 5 years. Must be Cancer free for 5 years.
7. Alcoholism or drug dependency
8. AIDS
9. Hepatitis C
10. HPV
11. Obesity with BMI Greater than 43+ | BMI Calculator
    
12. COPD.

There are many other declinable conditions. 
Note each carrier has it's own underwriting guidelines.
